A good lightsaber form for any beginner to learn is Shii-Cho. It's based off of Kendo swordfighting. Since lightsaber forms aren't real forms of combat, you kinda have to teach yourself. You could look around the forum to see if there's stuff on Shii-Cho, and if there isn't, do some research about Shii-Cho on wookieepedia.com, and do some research on Kendo. If you study both of them enough, you should be able to teach yourself Shii-Cho to a certain extent. It just takes time and practice. You should practice with a partner, by the way. No use in fighting an imaginary man.
You could either look at Sharper Image, Spencers, online, and possibly at CostCo. I know for a fact one of my cousins got his Force-Fx saber at CostCo.
I don't know about prices. It depends on where you're looking to buy 'em.
You can definitely practice the form with just a toy saber. Right now, I practice Makashi with either a toy saber or with my toy sword. As long as the instrument you're using isn't too short or too long, you'll be fine. Anything 4' or over is way too long in my opinion.

Marrik is right telling you about the forms on wookiepedia, but if you don't know anything about any kind of swordfighting, it won't help you much. Look up the points of contact first. It will help with the different strikes to the different body zones. Once you learn the zones, you can start with learning the strikes. Shii-cho is a very basic form. Simple sweeping strikes make up most of the attacks. You can look up katana forms on youtube, but most of the advanced forms are so fast, it will do no benefit. There is a chart of the points of contact, so if you have a sparring partner (if you don't, I'd suggest getting one) and practice SLOWLY with hitting the points. Once you get those down, you can learn to block them. From there you will be more comfortable with a sword and be able to learn other techniques easier. The biggest thing is having a partner to learn with or getting accurate with the body zones is going to be harder.
